Request a Police Report
Obtaining a Police Report
Do you need a police report? If you would like to request a traffic or crime report from the Fremont Police Department, please fill out the attached form titled, Application for Release of Information. All police reports are $13 and typically only available to individuals named in the report.  The form and $13 fee can be either hand delivered or mailed to the Fremont Police Department. A check, cashiers check or money order must be received in order to process the request.

Note:    This request form is for a Police Report which is different from making a Public Records Act request.   If you are requesting information via the Public Records Act, please fill out a copy of the  Public Records Request document and submit it to the City Clerk's Office.
